Why is the transmission on my Ford Galaxy not shifting smoothly?

I have a 2012 Ford Galaxy with around 50,000 miles on it. Recently, I have noticed that the transmission is not shifting smoothly. It seems to be hesitating when changing gears and is making a strange noise. What could be causing this issue and how can I fix it?
