How do I fix the squeaky brakes on my Ford Transit Custom?

I've noticed that my brakes on my Ford Transit Custom are starting to squeak when I come to a stop. Is there a way to fix this issue or do I need to take it to a mechanic? I've never experienced this problem before, so any advice would be greatly appreciated.
